Of course I have. Every once in a while, I have needed a mental health day (or it was beautiful weather and the golf course beakoned).
I never felt guilty for a second. Did my employer ever feel guilty for requiring me to work extra hours or weekends to get a "rush" project done (even when the rush was to get it completed in time to get the VP his bonus)? I think NOT.
My job now does not require regular hours or that I be in the office at any particular time. So, now I feel guilty if I am not working. Go figure.
Yes and No. Ever heard of a sanity day?
NOPE........BUT THERE WERE SOME

FEW NOTCHES SOUTH OF WORTHLESS.

Actually, I never had to ... my job included "Personal Days" as well as "Sick Days". I used personal days for my mental health and sick days for when Robbie was sick. Knock on wood .... I rarely needed actual sick days because I was sick.
The only things I felt guilty about were leaving my students to a substitute teacher ... usually not a good thing to do to a Math class.
YUP! Nope! They were "Mental Health Days". I worked with Mentally Handicapped Adolescents and Adults for quite a few years and one job had the whackiest schedule! 7 days on, 1 day off, 3 days on, 3 days off. By the end of that 7 day stretch, you were on "auto pilot". The staff sort of looked out for each other though. We worked 3-11 and you could tell if one of us was getting rough around the edges! LOL We'd usually tell them to go into the office, lock the door, and read a book and we'd call them if we needed them! LOL I'd never call off leaving any employer in the lurch though, sick or not. Many times I stayed and worked a double or triple during bad weather when overnight and day staff couldn't make it in.
I've done it for school, and then i stay home and have nothing to do all day.
